In a major step towards strengthening digital governance in public healthcare, the Government of Gujarat has launched the PM Family Care Tracker (PM-FCT) along with a Health Passport as a pilot initiative in the Gandhinagar Lok Sabha constituency. The initiative aims to ensure continuous monitoring of the health, nutrition, education, and welfare of mothers and children from pregnancy until 18 years of age. The platform integrates multiple government databases and creates a unified digital record for every beneficiary, helping authorities provide timely interventions and welfare benefits. The pilot project was launched by Union Home and Cooperation Minister Amit Shah and is expected to contribute to the vision of Viksit Gujarat @2047 and Viksit Bharat @2047.
The PM Family Care Tracker (PM-FCT) is an integrated digital governance platform that connects health, nutrition, education, and social welfare databases. Every eligible mother and child receives a unique digital identity linked with either the Birth Registration Number or ABHA ID, allowing authorities to monitor developmental milestones and ensure access to government schemes.
The primary objective of PM-FCT is to improve maternal and child welfare through technology-driven governance. It seeks to eliminate duplication of records, identify beneficiaries automatically, and ensure that no eligible person is deprived of healthcare or welfare services. The initiative also supports better policy planning through real-time dashboards and data analytics.
The platform integrates data from the Health Department, Women and Child Development Department, and Education Department into a single digital system. It tracks healthcare services from pregnancy through adolescence, including antenatal care, immunization, nutrition, growth monitoring, school enrolment, attendance, and adolescent health.
Automatic alerts are generated whenever vaccinations are missed, malnutrition is detected, or school dropout risks arise. These alerts help officials intervene quickly to prevent long-term developmental challenges.
The Health Passport serves as a lifelong digital health record for children and mothers. It stores important health information such as immunization history, nutritional status, medical check-ups, and developmental milestones. This enables healthcare providers to access accurate records instantly, improving continuity of care and reducing paperwork.

The digital platform will also improve transparency in welfare delivery by ensuring that government benefits reach intended beneficiaries without delays or duplication.
